Procedure
To make an appointment at the Denver
Adult Down Syndrome Clinic, please first contact the Interim Executive Director, Erin McLain, at director@denverdsclinic.org or
at (303) 762-6545. Once you have been given an appointment
date and time, please complete and return the DADSC
Intake Form. This form includes a health questionnaire, a release
of information form to establish previous medical history, and
a Report Distribution Form which lets you identify where you would
like the results from your Clinic visit sent. You may submit the
completed DADSC Intake Form by mail or fax. The Clinic fax number
is (303) 360-3713. Please bring the patient's photo ID and/or insurance
cards to your appointment.
Family members and/or care providers are welcomed and encouraged to join the patient during their appointment. Our experienced team includes Dr. Barry Martin, the DADSC Medical Director, and Heather Luehrs, the DADSC clinical social worker. The visits are scheduled for two hours which allows time for discussion and review of the patient's complete medical history, and assessment of the patient's current medical, psycho-social and emotional needs. Recommendations for further evaluations, referrals and care management will be determined and included in the Summary of DADSC Clinic Report which is distributed following the visit. There are laboratory and x-rays facilities located on-site.

Hours
The
Denver Adult Down Syndrome Clinic is currently open on Fridays,
two or three half-days days per month. Clinic appointments last
approximately two to three hours. Please plan accordingly.
