Procedure

After you fill out and submit the form on this page and we confirm your visit, click here to download the Denver Adult Down Syndrome Clinic's intake forms, which include a health questionnaire and a release of information form to establish previous medical history. Please fill these forms out and mail them to us prior to your visit. You may also fax the information to us at 303-360-3713. You will need to bring your insurance cards to your appointment.

During appointments, Dr. Martin, who specializes in the care of adults with Down syndrome, takes the complete medical history of patients and performs a thorough physical examination, and Heather Luehrs, our social worker, visits each patient. We perform laboratory tests and x-rays as needed and recommend consultations with specialists as appropriate to each patient's unique medical condition and needs. The Clinic will later mail you a comprehensive summary report.

Office Hours

The Denver Down Syndrome Clinic is currently open two half-days days per month, usually the second and fourth Wednesdays of the month. Clinic appointments last approximately 2-3 hours. Please plan accordingly.
