can seek help from the Denver Adult Down Syndrome Clinic?
The Denver
Adult Down Syndrome Clinic is a regional referral clinic designed
to provide medical and psychosocial evaluations for adults with Down
syndrome, usually 14 years of age or older. |

| What
services does the Clinic provide?
Our consultations
provide information regarding a patients healthcare needs, especially
medical needs that are more common among adolescents and adults with Down
syndrome. We also develop a psychosocial review as a part of each patients
consultation. |
| Will
my health insurance carrier pay for the Clinic visit?
Clinic expenses
often qualify under medical expense reimbursement plans. We accept Medicare
and Medicaid. Everyone should check with their primary care physician
to receive a referral.
The Clinic
will accept any and all adult patients with Down syndrome, who seek the
Clinics services. In no event will the Clinic turn away a patient
due to inability to pay. If a patient has no health insurance, Medicare
or Medicaid, and is unable to pay, the Clinic will address each patients
situation on a case-by-case basis. |

do I set up an appointment?
To set an
appointment, please call 303-762-6545 or email director@denverdsclinic.org,
and our staff will confirm your visit. |
| What
medical information do I need to bring?
You will
receive the Denver Adult Down Syndrome Clinic's Intake Form once
we schedule your appointment. This Intake Form will include a
medical history worksheet, which will identify health records that
you must mail or fax to the Clinic prior to your appointment
along with the completed worksheet. You will also need to bring your
insurance card(s) and a photo ID.
