can seek help from the Denver Down Syndrome Clinic?
The Denver
Down Syndrome Clinic is a regional referral clinic designed to provide
medical and psychosocial evaluations for adults with Down syndrome, usually
14 years of age or older.

| What
services does the Clinic provide?
Our consultations
provide information regarding a patients healthcare needs, especially
medical needs that are more common among adolescents and adults with Down
syndrome. We also develop a psychosocial review as a part of each patients
consultation.

my health insurance carrier pay for the Clinic visit?
Clinic expenses
often qualify under medical expense reimbursement plans. We accept Medicare
and Medicaid. Everyone should check with their primary care physician
to receive a referral.
The Clinic
will accept any and all adult patients with Down syndrome, who seek the
Clinics services. In no event will the Clinic turn away a patient
due to inability to pay. If a patient has no health insurance, Medicare
or Medicaid, and is unable to pay, the Clinic will address each patients
situation on a case-by-case basis.

We are located
at Aurora Medical Center,
700 Potomac Street, Aurora, Colorado. The Clinic is located
in Suite A.

| How
do I set up an appointment?
To set an
appointment, please call 303-360-3877 or fill out an on-line registration
form, and our staff will confirm your visit.

| What
medical information do I need to bring?
You will
receive a detailed packet of information once we schedule your appointment.
The packet will include a medical history worksheet, which will identify
health records, that you will need to bring to the appointment along with
the completed worksheet. You will also need to bring your insurance card(s).
Please contact your primary care provider to see if a referral is necessary.
